Our aim is to bring the best out of every pupil that attends The Pathfinder

Our SEND Team

At Pathfinder Church of England Primary School, we believe that every child deserves the opportunity to succeed in school. 

We work as a team to ensure that all pupils have the environment they need to achieve. Every member of staff is responsible for SEND provision in our school, the teachers, LSAs, midday assistants and office staff all play a part in providing learners with SEND quality first teaching and inclusion, liaising with the SENDCo and parents, to form a team around the child. 

We aim to provide relevant training to staff to support the children in our care. We work closely with outside agencies to support the children in our setting.   


If you have concerns about your child in relation to special education needs, you should share these concerns with the Class Teacher in the first instance.  The class teacher will then discuss these concerns with the SENDCo and the Headteacher as part of pupil progress meetings. 

All teachers engage in the Assess, Plan, Do, Review cycle half termly, and meet regularly with the SENDCo to discuss this and then with parents, where provision is shared.

If you have spoken to your child’s class teacher about your concerns and would like to refer your concerns to one of our SENDCos for further discussion, please complete a Parent SENCO Referral Form (see above) and send to: senco@pathfinderschool.org.uk


What does the local offer cover?

The local offer will include information about health and social care services, education, leisure activities and support groups in the area for children and young people aged 0 – 25 with SEND and their families.

There will be information about:

  • Services and support available
  • How children and young people’s needs are identified and assessed
  • The way schools, colleges and maintained nurseries support children and young people with SEND
  • How to access services and how decisions are made
  • Preparation for adulthood and independence
  • Arrangements for making a complaint and mediation
  • How to comment on the local offer

The Department for Education’s draft SEN Code of Practice and regulations outlines the range of information to be included in the local offer.

Why is the local offer important?

The local offer will help parents, carers and young people to see clearly the range of services and support that are available in their local area and understand how to access them.
